A few ways to attack 46 front.... And really depends a lot on what opposition is trying to do out of the front. I'll ASSUME that 46 in this case means alot of 8 man fronts and man and blitizing... Ima comment mainly on pre snap approach. A few things to get started.

(NOTE: I use to run Steelers QB draw pretty effectively against noobs who LOVED silver pinch or what ever the hell it was called, but I think that was before everyone learnd to set a DT to spy QB in these spots, ah wellllll)
Offer balanced offensive looks and count bodies in the box (ie look to left of center and count number of defenders and the do so to the right side as well). If they line up nice and even like, cool... play to your strengths. It's not 46 specific but if you plan on running between tackles or even off tackle its a nice approach.
Offer overloaded looks- I Wing is a nice look for this, motion FB to wing side (cant remember if madden allows this) and again, start counting, with a special eye for the A)numbers on front side perimeter B)numbers on back side perimeter and C) how the Corner (or whatever) is playing on the isiolated WR to the back side.
Offer tight looks- I tight is nice start. Shift to and/or motion to perimeter, make note if opp makes adjustments quickly or not. Above all TRY and make defense show hand in coverage and react accordingly. Be prepared to "max" protect and narrow down pass plays to one half of field.
Offer spread looks- 3-4 WRs has a funny way of breaking up dedicated 46 frnts... hehe... Depends on your particular approach to offense but I would always prefer to gash the Defense with a nice run up the gut than to worry about linebackers prowling the hook zone. Set audibles to where you have a pass AND run option out of the 3 or 4 WR formation that you run the play out of and again, count bodies to A) determine pass or run and B) Run direction.
When passing ALWAYS ALWAYS ALWAYS remember to set protection. This can be complicated (kinda) but you'll know it if the Defense is stacking the A gaps with some bleedin nano- pay attention and be prepared to run someone across the formation to insert into pass blacking scheme- a nice touch here is to smart route a RB into a delayed release pattern and run HIM from a WR spot into the protection. When in DOUBT stack the protection 3rd and 8 is MUCH better than 3rd and 15 (kinda?) PHEW!
Pay attention to down and distance (duh). But, seriously, 1st and ten- will I take a 6 yd completion to an isolated reciever? Sure. 2nd and 4, will I take a 1-3 yd gain right into the teeth of a Bears look? Absolutely. 3rd and 1? Youhave plenty of options to move the chains.... good luck
