Looks like a 46 bear or Goalline man blitz

From the looks of it, it looks like 46 Defense-Safety Fire.

I'm not sure in the setup but since the safety is blitzing on this play he controls the safety and brings him down.

i like the sam snake nano better than the safety fire but he is a dip s*** for attempting it that many times when it obviously wasn't working.
